3. Last date of receipt of application as per format given at our website Application as per requisite format along with self-attested photocopies of testimonials in support of Educational Qualifications, experience certificates, Attempt Certificate, Internship Certificate, Latest Medical fitness certificate from SEMO/CMO, including for Ex-Servicemen, PPO and Discharge book will be submitted to OIC, Station Headquarters (ECHS Cell), Kapurthala within 06 weeks for Serial No. 1 and 15 days for Serial No. 2 & 3 from the date of publication of advertisement in duplicate in sealed envelope. Any application received after due date will not be accepted.
4. Interview Date, Timing and Venue Candidates must reach Station Headquarters Kapurthala, Date and Time will be intimated telephonically. Candidates must bring all the original certificates/mark sheets/Matric, 10+2, Graduation, Post Graduation, Degree, Attempt Certificate, Internship Certificate, Work Experience Certificate, Latest Medical fitness Certificate and discharge book, PPO, service record and 02 PP size color photographs at the time of interview.
